- Replaced the algorithm used to build the texinfo nodes and menus. This was necessary to fix some bugs and to support the "@subsubsection", "@raise", and "@lower" commands. The major known bug was that single section chapters were not being processed correctly. Added the -c option so chapter names do not always have to included in the node names.
